Context

The mining industry faces significant challenges managing fatal hazard exposures, worker fatigue, and the complex interplay among people, processes, and equipment.

Challenge

A mining organisation sought a high-level review of their incident investigation strategies to determine possible process improvements.

Approach

ViVA conducted a desktop review of a sample investigation report and job safety advisory materials, also meeting with a lead project engineer. The review considered legislative and regulatory compliance, company policy adherence, scientific literature, and contemporary practice involving system design.

Outcomes

ViVA identified procedural strengths in the organisation’s investigative approach and identified six areas of vulnerabilities in their investigations, providing recommendations to resolve those improvement areas. This meant shifting investigations that focussed narrowly on procedural compliance to address systemic factors and work design complexity with a systems engineering framework.
